East Hazel Crest — Ch. 6 Art. VIII Registration of Multiple Dwellings § 6-143
Registration required
Operative Text
The owner or owners of any building shall file a registration statement for each such building with the village clerk on forms provided by the village clerk for such purposes. Any such registration statement shall be deemed prima facie proof of the statements therein contained in any administrative enforcement proceeding instituted by the village against the owner or owners of the building. The registration statement shall include the following information: (1) The name, street address and telephone number of each owner of the building. If the owner is a partnership, corporation, or voluntary unincorporated association, the statement shall further include the name, street address, telephone number and position of a responsible partner or officer. If the owner is a corporation, the statement shall further include the name, street address and telephone number of the registered agent thereof; (2) The name, street address and telephone number of a natural person twenty-one (21) years of age or older, designated by the owner or owners as the authorized agent for receiving notices of code violations and for receiving process in any court proceeding or administrative enforcement proceeding on behalf of such owner or owners in connection with the enforcement of this code. This person must maintain an office in Cook County, Illinois, or must actually reside within Cook County, Illinois. An owner who is a natural person and who meets the requirements of this subsection as to location of residence or office may designate himself as agent; (3) The name, street address and telephone number of the owner's agent for the purpose of managing, controlling or collecting rents and any other person not an owner who is controlling such buildings, if any; (4) The name, street address and telephone number of each lending institution or party holding a mortgage on the property, if any; (5) The street address and property index number(s) of the building, and the number of family units therein. For purposes of this section, a post office box does not suffice as an address.
East Hazel Crest § 6-143 requires the owner or owners of any multi-unit residential building to file a registration statement with the village clerk, using village-provided forms. The statement must identify the owners, a designated local agent for receiving legal notices and code-violation notices, any property manager, mortgage holders, and basic building details. Once filed, the registration statement can be used as prima facie evidence in any administrative enforcement proceeding the village brings against the owner. Post office boxes are not accepted as addresses anywhere in the form.
Plain English — not legal advice.
Under § 6-143, compliant building owners file a complete registration statement with the East Hazel Crest village clerk for each covered building, using the official forms provided. The statement must name a designated agent who is a natural person at least 21 years old and who either maintains an office or actually resides within Cook County, Illinois — a P.O. box does not satisfy the address requirement. Operators who own through a partnership, corporation, or unincorporated association also include the name, address, phone number, and title of a responsible partner or officer, and corporations additionally identify their registered agent.

§ 6-143 establishes a public registration system for multi-unit buildings in East Hazel Crest, which means tenants may be able to look up who owns their building and who the designated local agent is for receiving legal notices. If a building is not properly registered, that fact may be relevant in administrative enforcement proceedings the village initiates. Tenants who believe their building is unregistered or that the registration is incomplete can contact the East Hazel Crest village clerk's office or a local tenant-rights organization to understand what options may be available.
